I'm no expert, but I think you should say "O namae wa nan desu ka ?" (polite) or "namae wa ?" (casual). Try to use "anata" the less as possible, people really never use it in Japan, it sounds really weird. And you can almost always omit the subject in Japanese if you can still understand who the subject is from the context.
